ESA Supporting Evidence was created by Telly
Hello

When submitting supporting evidence from health professionals with the ESA form, who should you ask that they address the letter to?
Is it To Whom It May concern?
you?

I have been looking but cant seem to pin it down.

Thanks in advance.

Hi Telly,

Evidence can be addressed 'To whom it may concern' or to the 'Decision Maker', whichever you prefer.

Nothing on this board constitutes legal advice - always consult a professional about specific problems
