Just received this form for my step son who is currently on IB, we cant recall when he last had an assessment for IB. I've just downloaded your guides for this. My Step Son hasn't needed to see his Doctor much for years really, as despite him being very severely disabled (Hydrocephalus, Cerebral Palsy & Epilepsy) being Brain Injured & Wheelchair Bound, his day to day health is quite good.
I'm sure I read somewhere that you had a form for people to send to their Doctors, but I cant find it in the guides & i'm not sure what the form was intended for, could you advise on this.
Also, where it asks for details of care professional who knows you best, it indicates that a personal assistant would be suitable. I am my Step Sons PA & he employees me via ILF payments as me & his Mum are separated so he doesn't live with me. Could I complete this part of the form oe would I be best just sending in a letter with the form as a Carer/Relative?

Hi Taggy,
The form that you mention is an ESA113 that the DWP (occasionally) send to the claimant's GP. It is not generally available as it is not meant for use by the claimant. To be honest, the form is so limited that it is not much help anyway, you would be better asking your son's GP to write a letter of support (refering directly to the descriptors that apply to your son's impairments) if s/he is prepared to do so.
The 'care professional' section is really for professionally qualified health professionals, you could write down the name of a Social Worker or Physio for example.
As your son's daily carer/PA you can also send in a statement in support of his claim, you should state that you are formally employed as his PA and thus have intimate knowledge of his limitations.
