Have not recieved an ESA50 to fill in and finding it rather strange i have not been consulted for a suitable time and date. Must go on the date given or my benefits will be affected, anyone else experienced similar. Will be consulting my doctor on what should be my course of action. Rather worried and confused.
We had another member in the same situation a couple of weeks ago.
Are you already on ESA or is this a transfer?
If a face to face assessment is required then an ESA50 should have been issued, although it is possible that it was sent but not received.
Providing you have had seven days written notice, they use the date of the appointment letter, then I do not think that there is any opportunity for you to avoid attending, however, I would contact the DWP and explain the situation, you may need to escalate the problem to a Customer Manager.
I would also contact ATOS in similar vein.
Phone: 0800 2888 777 or 0113 230 9175

Given the timescales I would phone them, but follow it up in writing.
In all likelihood there is not time for you to complete an ESA50 before the assessment, and it is something that should be completed in detail rather than in haste, but if you want to have a go at it then there is a downloadable version available.
Using The PDF Version Of The ESA50 Questionnaire
Whether you intend to complete the ESA50 or not, I would also recommend that you have a look at the ESA Claim guides, the ESA Descriptors and Regulations were updated in January.

I am more tempted to go with incompetence, but I guess the two are not mutually exclusive
