I have just finished completing my ESA50. For one of the questions, i.e. the 'Moving around' one, I have decided not to tick any of the descriptor boxes but have gone on to describe my difficulties etc in the large box provided and have continued on to a separate sheet too...
Would it be advisable to put a note by the side of the unticked boxes saying something like 'please see below' - to try make it clear that I meant to leave the boxes unticked and haven't simply forgotten to tick one of the boxes?

Yes, you could do that, or write something like " no tick box options applicable, please see further comments below, and on added pages number xx".
