As far as I am aware your appeal is not dependant on you producing a Fit Note and so should continue, however, without one you are not entitled to any payments while you wait for a hearing.
Did your GP say why they will not issue one?
The DWP of often send a letter to the GP following a Fit for Work Decision, saying that they no longer need to provide Fit Notes, unless the claimant is appealing.
The issuing of a Fit Note is a medical decision and should not be affected by a benefit decision.

There is no administrative reason why a GP cannot issue a Fit Note for these conditions, perhaps you should see your GP again, or consider seeing another one, although in this case they are unlikely to issue a Fit Note in the short term.
