I am not sure whether or not there is an interactive DLA Claim Form, maybe one of the other Mods could confirm this.
However, if there isn't, you could always write on the DLA Form please see added Appendix 1, 2, 3, etc, etc, and type out any comments on separate A4 pages.
If you were to do this, as with The ESA50 and added extra pages, put your name, NI Number and DOB on all extra pages.
