Question about adding pages was created by cackeyedlil
Hi I hope you could help me, I have been trying to fill in my ESA form and the boxes are far too small for the answers I need to give. I know we can attach extra pages, but where do we attach them and how can I make sure that they read and include them while assessing each question. Should i attach them to each page, so i.e. staple andextra bit to the standing and mobility page. I'm worried if they're all clumped together at the end some Jobsworth won't read and include them. Thanks and thanks for the guides

To be honest, if they are not inclined to read them then printing them in a 72 point font will not make a difference.
There are positives and negatives to both approaches.
By attaching them to the page you make it more accessible but providing reference numbers and page numbering can then be a problem.
Attaching them all to the back is less accessible but attachment may be easier and certainly indexing numbering will be more sensible.
The page being more accessible is not necessarily an advantage because it also more easy for the reader to return to the form, there is an argument that if they have to go and look for it, then they are more likely to read it.

Hi
I put "see attached sheet "in the relevant box ,then added the extra sheets on the same page as box.I stapled them on.You need to make sure each page is numbered and your name and NI number is on each one.
It worked in my case as I was placed in support group without a F2F .Good Luck
