I have received an ESA3 form to complete today after returning my ESA50 on the 18th February. Are there any guides to completing this form please as I wasn't sent one by the JobCentrePlus office?

This form asks about your income, savings and assets and the hours that your partner works, just answer the questions and provide any information requested.
