ESA forms: repeat forms for continuing claims

I have been sent another ESA form ( ESA50 03/11). I get NI credits but no paid ESA since the maximum term was reduced to 1 year for contribution based and my first year exoired some time ago, but I want to carry on getting NI credits. I am not sure which B&W Guide applies to this form? It is not my first ESA claim, but it is on the ESA 03/11 form. I should have sent it back in January 2013 but have been too exhausted with ME and DLA renewals to face it again. Can anyone tell me which guide should apply to this form?
