Well after nearly three years in the support group an ESA50 form has popped through the letter box.
It must be time to be re-assessed!
Have downloaded the new guide and hoping that it will help making completing the dreaded form easier, as the old one did all those years ago.
Any advice welcome.

If it is the new ESA50 01/2013 that you have received, you may wish to consider :
Using The PDF Version Of The ESA50 Questionnaire
(If it is the old ESA50 03/11 please let me know, and I will post a download for that)

Just a brief update.
Now six weeks since sending in my ESA50 form and nothing heard yet.
No news is good news???
