It states that i will recieve a call and form shortly, to complete. Though i was unsure if i still require to complete this as i currently recieve high rate care and mobility. My renewal and transfer to pip is not untill 2015.
I would be grateful in any help, advise with regards to my situation, im planing on cross reffrencing from my claim for dla which i just completed in 2010, along with notes from previous ib renual for 2010, also. Along with your helpful resources.
Many Thanks,
Kind Regards,

The phonecall is just to tell you about the transfer and make sure you understand what to expect. If you do not take the call, the ESA50 form will be sent out anyway.
I'm afraid that there is no exemption from completing these forms even though you receive DLA. You should make full use of our ESA guides when filling out the form.
ESA claims and appeals
We also have a lot of FAQs dealing with the transfer process, it is worth reading them all
IB/IS/SDA transfer FAQs
If you wish to get a head-start you can have a look at the PDF form
Using The PDF Version Of The ESA50 Questionnaire
I'm sure you will have further questions as you go through the process. Please ask them on this topic/thread as it helps us if everything is kept in one place. You will find it easily in future if you mark it as a favourite, or bookmark it, now

It is much appreciated, thank you for posting links to direct info, regarding the transfer claims and appeals along with the online pdf form. I may find it easier, cut paste etc to complete within the timeframe.
I no doubt will have some more questions as i progress, as the form has changed since i completed the past 2 Pca forms, which was on the old point system.
Thanks for the advise on linking the threads, and bookmarking and favourite, the site has changed alot over the years, though this new layout is much better.
Many Thanks again,

You should be aware that the ESA system bears little if any similarity to the IB system that it replaces. there is a guide in the ESA section that describes the differences.

Ive since received my form with a july deadline, for handing in.
I was unsure what guides to follow as, i see that there has been two different forms along with two different rules and some may be applicable.
My Ib was for review in 2012, though have not received, a form until now it is the new form with the esa5001/13,
Im slightly confused with the changes, as my form has to be handed in few days before july changes, when i am assessed will i fall into the old rules category, or will i now be assessed under the new system.
What is the difference with the old rules to the new ones.? for appeals etc
Sorry if im longwinded but i was just hoping someone to help, before i make a start, with some headway with the forms.
Many Thanks,

As it is the new ESA50 dated 01/13 then it is the Guides marked new ESA50 form
There is an explanation of the changes in the guides.
As the new Decision on your claim will be based on the current "rules" so would any appeal that you might make.
