Hi all, I need some advice please. Firstly, my husband is filling out the ESA50 questionnaire for me on the computer. I know that ATOS can contact you over the phone about attending a medical but do you have to take the call? I ask this because I can't cope answering the phone, so hardly ever do, therefore can my husband put in the questionnaire that I would not want them to ring me up but to write to me if I needed to have a medical?

Secondly, on page 19 of the questionnaire it says about ticking a box if you are including any medical reports, does this mean supporting letters off family, etc.. or not?

Hi Rosie,

You do not have to take any phone calls if you do not wish to do so, you should mention on the form your desire to undertake all communication in writing. However, you might find that DWP try to phone you anyway!

Appointments for the face-to-face assessment, should one be required, are usually sent out by letter.
