Hi all, apologies if this has already been covered else where on the forum.
I have had my renewal DLA form through and i am slowly working my way through it.
My health has significantly deteriorated since my last claim, i now have physical disabilities as well as mental ones.
Had to have an OT assessment who have provided myself and my husband (my carer) with aids to help around the house with my care and mobility.
My question is.... do i ask the OT for a copy of their report on me? and /or a supporting letter to state what mobility problems they have seen. and should i send it with my renewal if the OT is prepared to support my claim?

Hello K,
Yes, it is worth getting a letter from the OT, or a copy of the assessment form, and sending this in support of your DLA claim. Don't forget that you should treat every reassessment as if it was a new claim and provide whatever evidence you can in support.
On another note, it appears that your forum name might be your real name. Many people prefer to use an alias in order to maintain a degree of anonymity. if you wish to change your username you should follow the advice in this FAQ
