There is no reason why you should not mention it on the form or any attached pages.
Have a look at the ESA Claim Guides for a more detailed explanation of what you will need to cover in any statement in regard to this. There are two Exceptional Circumstances Regulations; one for the WRAG and one for the Support Group.
