Splitting benefit application forms

There is only one DLA renewal form and this covers Care and Mobility needs resulting from both Physical and Mental Health grounds.

We provide two guides as the approach for the two is quite different and it would be confusing to try and put them in one guide, all you need to do is include both your physical problems and any MH ones in your answers to each question.

So as a simple example; I have problems going out as I cannot walk because of severe discomfort in my legs I also have issues going to places I have not been before because of anxiety.

You should treat the renewal as a new application and include full details of you problems and provide evidence to support this as available, do not assume that the DWP will use any information that you have previously sent them.
