Medical Input to send with form

The problem is that ATOS seldom send ESA113 Questionnaires to claimant's G.P.'s
You could request copies of your G.P. records, and use any relevant extracts from those as evidence :
Requesting your medical records
Charging for medical records
Have a read through this document :
"Being transferred from incapacity benefit to employment and support allowance. A 22 page guide to the transfer process."
At the top of this page :
ESA Claims Guides
As you will see you first receive a letter, then a phone call, (which you don't have to take) then the ESA50 Questionnaire.
If you have received the ESA50 Questionnaire, then you must fill it in and return it to ATOS by the deadline given.
You may wish to consider :

Whilst referring to the ESA Claims Guides under the heading :

Just to add, it is important that your ESA50 is returned by the deadline given, otherwise your benefit may cease.
If you receive any further evidence after that, you can post it on to ATOS at the same address.

However like I said in my question the stumbling block is proving getting the necessary medical input and I did tell the doctor that it is very unlikely Atos would send GPs requests asking for info on claimants once the form is being looked at. Thats why I pressed the point to him that anything at this stage to accompany the form would best help me even a brief report which I would naturally pay for. I think the only route I shall have to go down now though is to request my medical records and then sort out the relevant information for myself for my claim. However time is not on my side as the doctors have already dithered around for a week deciding whether they would or would not write something for me. Thank you for your rapid response.

Unfortunately, more and more G.P.'s are taking this stance.
