A recent ESA award letter gave an additional £58.20 a week, described by DWP as "extra money because you are severely disabled" Under what circumstances is this awarded?

Hi c,
To gain an exact answer you would have to contact The DWP.
It could be an IR ESA top-up as you are entitled to Disability Premiums.
If you are in receipt of MR or HR Care DLA, then the minimum amount that the government states that you need to live on, increases.
Therefore, any income that you may get from benefits, etc, is topped up to this minimum disability income guarantee.
So, as you should understand, depending on your own personal circumstances the amount that you may receive as a DP (IR ESA) would vary from person to person.
