Letter About IB Change Over To ESA

I had the letter Monday morning, and have been worrying myself stupid since then, what does it intail and do I need to talk on the phone? I'm not good on the phone, I'm worried about the forum, and I just want to curl up into a ball and cry. I knew this letter would be coming, but didnt realise it would be so soon. I think I'm going to need some help, but other then CAB I dunno whom to ask .

As you will see you first receive a letter, then a phone call, (which you don't have to take) then the ESA50 Questionnaire.

Once you have received the ESA50 Questionnaire, you must fill it in and return it to ATOS, by the deadline given.
