Two Questions Please

I know we have to include our name, D.O.B. and N.I. number on each additional page of information we send in with the ESA50 01/13 but do we have to add them to the pages of the PDF version when printed?

If I print out the form on single sides, can I use the reverse of the sheet which contains the question I need more space for?

e.g. page 13, question 11. ‘use this space’ full so continued on reverse of page 13. Obviously giving notice of having done so. The page 17 they say to use is already used.

Hi Susan,

Yes, if you have separate pages then it is well worth putting your name, DOB and NINo at the top of each one. It is also a good idea to number each page '1 of 100' (for example) as it is then plainly evident if something goes missing.

If you want to use the back of the page for additional information, that sounds fine to me.

Sorry Mrs H.B. I mean do we put our name, D.O.B. and N.I. number on the actual ESA50 ? The online version, even though, apart from colour, it’s the same as the original pink one.

Yes, as the sheets of paper will be loose, you should put your details on each one, even if you have managed to fasten them together somehow. I would not suggest this for the pink ESA50 as the sheets are securely fastened together.
