Sending PIP Forms

I used the PIP guide and have filled out the application form , even though it took best part of 2 hours, put in as much info as I can , however I don't really have any supporting docs to send. I have letters from consultants I get , when I go to clinic, they basically write with what we discussed and what is planned next.
Do you think I should send them as supporting documents, as I'm not too sure on what to send and it asks on the first page, what documents I will sending with my application ,also I don't know if a copy of my prescription would be useful ?
Any advice would be appreciated.

Evidence for PIP claims is covered on page 57 onwards of our PIP Claim guide

As a general rule anything that indicates that you are receiving treatment is of value, so I would certainly consider including the documents you mention with your claim form.

Do you think , I should enclose a copy of my blue badge, to show that i cannot walk long distances and need parking close to buildings and entrances ?? or does this not really be taken in to consideration ??

I would certainly include details of your Blue Badge award as it shows that you have mobility problems, even though the assessment criteria are different, whether you should include a copy of you actual badge is up to you.
