Hi bro 58 thanks for your kind reply I will try to get to grips with this forumbut finding it hard!!! I am looking for help on ib to esa questioaire forms I have a long term illness which is classed as physical by WHO but have learned in Jan 2013 that I was diagnosed with a mental health diease caused by chemical posioning whilst at work from 1990 dia 1997 but was lied to about the truth!!! Something the goverment would like to sweep under the carpet and am worried this may affect my ongoing benefit cheers mark

O.K. Mark,

So you have now grasped how to pose a query to your topic thread.

If you now Bookmark or Favourite this page in your internet browser, (usually done at top right of your browser) you will have a link to find it again quickly.

It helps the Moderators if you can keep all your queries to this one topic thread, as we can then see what advice has been given previously.

Firstly, qualifying for ESA is a lot different to IB, so you may like to start by reading the following document :

Further down, on the same page.

As you will see you first receive a letter, then a phone call, (which you don't have to take) then the ESA50 Questionnaire.

If you have received the ESA50 Questionnaire, then you must fill it in and return it to ATOS by the deadline given.

You may wish to consider :

Using The PDF Version Of The ESA50 Questionnaire

Whilst referring to the ESA Claims Guides under the heading :

"New ESA50 form"

On the same ESA Claims Guides page as above.
