I found a DWP Subject Access Request Guide online, although it dates back to April 2008. Optional paragraphs can be inserted into a SANTA01 for purposes of identification and clarification (and prevarication?). These include requests for a current utility bill and evidence of power of attorney. Looks like they just threw them all in.
Perhaps all I need (since I am in the SG this time) are the ESA85a and LT54 forms? My GP wrote a letter to go with my ESA50 and the prognosis and information about descriptors will appear on the forms anyway. Won't it? Am I over thinking this?
BTW, in response to Bud's reply, there wasn't an email address on the letter.

Hi SCH,
From what I can see of the quotes from your SAR request, there does not seem to be anything untoward with them.

Did you include dates From-To ?
If you are not happy, a threat of a complaint to ICO may help

No, I didn't put any from/to dates. The form includes these of course. I will repeat the information I previously quoted above and write a covering letter re no need for Power of Attorney and get it sent off (they only gave me 20 days to respond and the original request took 10 days to reach them apparently despite being sent by recorded delivery).

Hi SCH,
It may help if you address the envelope, FAO : The Data Controller, and write DPA 1998 SAR in front, top , left of envelope.
