ESA and limited capability for work questionnaire was created by weymouth321
I have my 2nd ESA - limited cap` for work questionnaire arrive, after 2 yrs. I have rheumatoid arthritis. ( Was dismissed from my job, on incapability, due to ill-health ; July 2011 ). I am 56 yrs.
I have had 2 face to face medical assessments, where I have been placed in the ESA - WRAG Group.
I have to return it by 10 th Sept. Does one always have to have these face to face medical assessments when in the ESA Group ?

Hi Josie,
No, not everyone has to undergo a face2face assessment if ATOS feel that they already have enough information, on the ESA50 and any other evidence submitted, to make a decision.
Please be aware that the ESA descriptors changed last year so make sure you take this into account when filling in this ESA50
ESA claims and appeals
If you prefer, you can use the PDF version of the form
Using The PDF Version Of The ESA50 Questionnaire
If you have any further questions, please come back and ask them on this topic/thread as it helps us if all information is kept in one place. You will find it easily if you make it a favourite on your web browser, or bookmark it, now
