You can write to DWP and tell them that your condition has worsened. This will result in a new claim form being sent to you and you should treat this as a new claim and not presume that DWP have any prior knowledge.
As DLA is now closed to new claims, I think you would actually be sent PIP forms so you should have a good look to make sure that you think you will meet the PIP criteria. (I'm sure one of the other mods will correct me if I am wrong in this).

My previous claim DLA which I am still receiving is nothing to do with this illness now and they have sent me a form (DLA434 looking again at a claim,) disability living allowance. So what I really wanted to know does this form have to have all previous illnessess aswell as my new illness which is making everything worse in order for me to try and get a higher rate.
I have read somewhere that applying for this is not easy.

I see that Gordon is going to give you more information, so I will not comment further at this stage.

You should treat it as a complete re-claim, also you should not assume that the DWP will review your previous claim forms, so you need to complete the form in detail and supply evidence to support not just the changes in your conditions but the reasons for your original claim.
