Thank you for reply. Could I ask you, does this mean I need to get the copy of my old claim and repeat it on to this new for m in full, also adding new illness, medications, and new problems and letters from support.

I would certainly re-visit your previous/original claim form, the important thing is to complete all of the sections of the form that are relevant, even if they are related to an area that has not deteriorated, but I would look at your answers in the context of the latest DLA Claim guides, in case you can word them better or more effectively.
