ib to esa50 new form

ib to esa50 new form was created by bal42
new esa50 form completing

my friend has almost completed his form but he has TICKED all questions

example is :-- please tick this box if you can move around and use steps without difficulty

he ticks in the little white box now go to question 2

then he ticked on 100 meter white box and completes details in large space box

he ticked questions like this where it says -- NOW GO TO QUESTION 2,3,4,5,6,9

WHAT CAN HE DO TO RECTIFY ALL THESE TICKS ??

can he tipex these ticks

or cross them


any kind advice please and many thanks in advance

Hi b42,

Tipex could be used, however it would have to be clear which tick box options, if any where being chosen.

Another option would be to redo it using the Pdf ESA50, which can be edited and saved as many times as needed.

This is one of the advantages.

he has chosen the right tick box options according to his limitations but he also ticked all questions first little box where it says now go to next question. this could be bit confusing to the reader .

ticking now go to next question means this question is not applicable to me and at the same time he is ticking the option boxes whic apply to him and completed the details box.

Hi b42,

As I have said, if you are to alter the ESA50, just make sure that it is clear to the ATOS assessor which options have been chosen, and which have not.

one more question --can he attatch note with the form stating he has ticked these boxes by mistake and should be ignored
