I have an appointment on Wednesday for the Endochronologist, which is our first meeting, so he won't be able to fill them in with anything much.
Will copies of my blood tests do or should I not bother with these? Afterall, they only mean something to someone who knows how to read them.
Many thanks

However, I only have my blood test print outs which have all my information on including NHS number. I have recently had my doctor who was dealing with me leave and the new doctor just rushed me through and left me in tears when I left the surgery. She doesn't have much idea about what I have suffered.

Hi LL,
You could include the "Blood Test" with references to reputable information (from the internet) as to what the results of the Blood Tests confirm.
The NICE and NHS sites may be a good starting point.
Remember to qualify for The WRAG or SG, although it is important to provide evidence that you do suffer from any particular medical condition, to qualify, you must also show the the resulting limitations fulfill the necessary WRAG/SG Descriptors.
Qualifying for the WRAG
Qualifying for the Support Group
WRAG (LCW)Schedule 2 Descriptors
SG (LCWRA)Schedule 3 Descriptors
ESA REG 29 :
ESA Reg 29 for entry to The WRAG (LCW)
PLUS Post 28/01/13 Amendment of :
(3) Paragraph (2)(b) does not apply where the risk could be reduced by a significant amount by—
(a)reasonable adjustments being made in the claimant’s workplace;
or
(b)the claimant taking medication to manage the claimant’s condition where such medication has been prescribed for the claimant by a registered medical practitioner treating the claimant.”.

ESA Reg 35 for entry to The SG (LCWRA)
If the Endocrinologist will give written confirmation of your diagnosis, that will help.
If the Endocrinologist, will provide written support with respect to how and why your resulting limitations qualify you for The WRAG/SG whilst referring to the relevant descriptors, that would be even more helpful.
You should not miss your deadline to return the ESA50. If you receive any further evidence after that, you can always forward it to ATOS at the same address, and to The DWP ESA DM, with a covering letter asking that the evidence be taken into account in addition to the ESA50.
Send copies and keep originals.

I actually suffer from Morbid Obesity which I can't shift due to the hormone issues. I have several mobility issues and was placed in SG last year without a formal diagnosis. I have worked through the form and I am still under medicated as I suffer many clinical Hypothryoid symptoms and not reached optimum levels. I am also a Hashimoto's sufferer which is an autoimmune disease.
I know I fit the criteria - I wish I didn't have any of this to be honest with you, but I do.
I'll print out some stuff off the net - thanks

No problem LL,

For future reference,
When you see your Endocrinologist on Wednesday, you could make a request that all future correspondence that your Endocrinologist sends to your G.P. post hospital clinic or as the result of tests carried out is copied in to you.
This is permitted, there are no forms to fill in and no cost involved.
If you have any problems with this request contact PALS at the hospital, they will sort it out for you.
