Query: Copy of form to DM

In the coming months I shall send an ESA 50 to ATOS. I plan to follow the B & W guidelines and forward a copy to the DWP Decision Maker. A covering letter is appropriate, I think. Is there a template available?

Hi MW,

There is no template for this.

Just explain that you have sent a copy to the DM to ensure that they have access to exactly the same evidence that the ATOS HCP has, as an aid to the decision making process.

I shall do that on the ESA50 sent to both ATOS and the DM. ATOS expect the form the DWP do not. Therefore, a covering lletter to explain the arrival of the copy is necessary. Whatt should I do? Just compose a brief letter and attach it to the form?

Yes MW,

As the shadow copy of the ESA50+Evidence that is sent to the DWP DM is done by choice and not obligation, you should put the covering letter in the envelope/package, with the above.

It will then make it clear to the DM why you have sent the shadow copy to them.

As you rightly state, there is no need for a covering letter to ATOS, as they will be expecting the ESA50+Evidence, as return of this to them, is obligatory.
