NO ASSESSMENT TOOK PLACE

This was after waiting for an appointment after fillling in my esa50 back in September 2013.
I am being switched from Incapacity Benefit .
I brought along a new updated ESA50 as there has been some changes I would like considered on my application.
After a few questions the assessor could clearly see that my various medical problems some of which are very rare, were way beyond her comprehension and she told me that she thought it best I should see a Doctor.
I offered her my updated ESA which she refused and told me to post it off with a covering letter explaining what had happened .
I have done this and sent this information to the sddress showing on the appointment letter which is Centre For Health And Disability Assessments, Government Buildings, Brislington, Bristol.
Is this a Maximus address or is this a DWP address, and where should I send this new information to.
I am concerned that I may have not sent this information to the correct people.

You need to send it to Maximus at the same address as your aborted assessment, I would also send a copy to the DWP office dealing with your claim, marked for the attention of the Decision Maker and you should again take a copy to any re-arranged assessment, it might just be excusable them not taking it this time because they did not actually do an assessment but assessors are required to accept and read any new information that the claimant provides on the day of the assessment before they start the interview.

The medical was at Truro in Cornwall, do you mean I should send a copy to them as well as the DWP office.
The person doing the assessment would not look at it.
How do I find out the DWP office, is there more than one?.

For the letter to The DWP ESA DM, it would be the address of the Benefits Office/Centre on your most recent ESA letter.

The above is the address for : The Centre for Health and Disability Assessments (CHDA) (Maximus) who took over the ESA WCA Contract from ATOS on 02/03/15.
Sending Documents to the DWP or Maximus (CHDA)
The Centre For Health And Disability Assessments, (CHDA) is Maximus.

I have just rang the freephone number on my esa appointment letter 0800 2888777 and was told to send my new updated esa50 to them ONLY.
I mentioned I was advised to send it to The Decision Maker at the DWP office and I was told incorrect.
I was told that they must (Maximus) otherwise known as Centre For Health And Disability Services, Government Buildings, Brislington, Bristol, BS4 5LA must see any new information first and it is them that would advise " The Decision Maker " on what the next steps were to take.
What am I to believe? .

Hi TR,
Maximus are correct in that it is they who will look at any medical evidence that you may present and then make recommendations to The DWP ESA DM as a result of said evidence and any findings as a result of a face to face assessment if one is carried out.
However, Gordon has advised you to also send copy to The DWP ESA DM to ensure that they have access to exactly the same evidence as Maximus when they are coming to a decision.
Therefore, in theory, you do not have to send a copy to The DWP DM, doing so is purely a "back-up" exercise.
If you don't want to send copy to The DWP DM, you don't have to, it's your choice!
