" For the letter to The DWP ESA DM, it would be the address of the Benefits Office/Centre on your most recent ESA letter.
I have never had a letter from the Benefits Office/Centre, only the letter of appointment on the 15 from Maximus in Bristol.

How do you contact the DM at the DWP when you have had no correspondence from them and only your appointment from Maximus.

Hi Tony,
Whilst the DMs do sometimes phone the claimant, it is not possible for the claimant to phone and speak directly to a DM. The best you can do is phone the DWP call centre and ask for a call-back from a supervisor. These people are usually more knowledgeable than the people who initially answer the phone.
Edit - Don't know why I read this as 'phone' when you clearly said 'contact'!

Phone the ESA hotline for the address
THE ESA PHONE NUMBER DWP : 0345 608 8545 Press Option 3 for ESA. (Open 8AM-6PM. Mon-Fri.)
