ESA50 DOWN LOAD ?

Bro58...Many thanks for your quick response and the link which was great. I think the following paragraph is really important. It had ever occurred to me that the assessor might be starting from scratch! It should be on the front page or somewhere really prominent.

"You should treat this re-assessment as if it was your first assessment and not assume that The Assessor will have access to any of your previous evidence. They may not even know that you are currently in The SG."

So here we go with the seven extra sides and I think I'll send the review report as well

I did the printable version and filled it in not in capitals as it was word processed. I found this easier and also you can save a copy on your computer

I am too worried to send it like this so i will print it off tomorrow, i will i think have the room to be able to add quite a bit more information to it too,

Should i tell them i am in WRAG but am sure i meet a couple of the descriptors for the support group since my last assessment 5 years ago?
I don't want it to sound like i am trying to tell them their job?

I have instigated this assessment by telling them how badly my health has gone downhill since my last assessment and sending them a GP's letter,
But right now I'm wishing i had left everything well alone, though i couldn't really as the job center lady did advise that i notify them of my badly deteriorating health and new conditions

If you instigated it through a GP letter, please ensure that you include another copy of the GP letter with your ESA50 AND refer to the GP letter in the relevant sections - i.e. where your health has deteriorated.

I.e. Do not assume that the ass' will actually have a copy of that original GP letter.

Hope it goes well and that you are doing well.

I personally would recommend using Voice Recognition if possible, so that the results are printed & they cannot say that they haven't read it because it is illegible.

If you do have to hand-write in CAPITOLS and it caused pain and stress - then say so at the start of the hand-written piece.
