do you actually mean tribunal?
Have you had a MR and appeal?
If not you need to request a Mandatory Reconsideration as the first stage of appeal.
You can write requesting it within a month of your decision letter.
If you have had the MR upholding the decision you go to appeal which should all be in the decision letter. On the appeal form you request a face to face or paper hearing.
The home assessment is only for the initial claim.
As you have the decision you have passed this stage.
Have you read our guide to appeals in the members area? This explains everything.

You've confused us with your post about a home assessment :(

Do you mean a benefit Assessment in which case you need to contact the Assessment Provider, you will need a letter from your GP explaining why the claimant cannot attend at their offices?

OR

do you mean a domiciliary Tribunal Hearing? If this is the case then you need to include a letter with the appeal request form explaining why one is needed. Again, a letter from your GP would be required. You do need to be aware that this type of hearing is now quite rare, as alternatives you can offer a hearing over Skype or a similar video conferencing application or over the phone.

Welfare Worker in the context of an appeal has a special meaning, it would be someone acting as a Representative (big R) for the claimant, you can act as their representative (small r) if you want without problem. The difference between the two is down to the expectations of the Judge as to the actions that the person will carry out, you would be doing the claim a disservice if you applied as a Representative when you are in fact a representative, if that makes sense.
