I've had two letters concerning my ESA50 - both have Health Assessment Advisory Service letterhead. (1) The first letter (that came with the form) asked me to return the form to them in the SAE to their office in Wolverhampton. (2) The second letter was their reminder, asking me to return the form to them at Edgbaston, Birmingham. (I sent it to Wolverhampton with a letter pointing out the different addresses they gave me.)
There is a local DWP Jobcentre office in the City where I live. This building also has a Health Assessment Advisory Service assessment centre there (which is only accessible by lift or stairs).
Everything has become so confusing & elaborate since the Government's attempts at re-organising welfare.

You need a letter from the DWP, you should have received an entitlement letter or something similar.

Do you mean the letter that told me I was entitled to be in the Support Group back in 2012?

Ah - are you referring to the ESA letters that are sent periodically letting me know how my ESA has been worked out and notifies me of the Christmas Bonus Payment? Those are on jobcentreplus letterhead from Wolverhampton.
