I'm sorry, you can argue the semantics of it but you have been classed as failing to attend the assessment, attending at the centre is insufficient, you must also participate in the assessment but you set an unreasonable requirement and as a result of this you were classed as not participating.
You could have recorded the assessment, if you had met the requirements that the DWP set for this, but from your post, you chose not to meet this requirement.

For ESA, Maximus could have provided the equipment and made the recording for you, you chose not to use this service, you could have made the recording to two relatively cheap tape recorders, I'm not suggesting that this would not be an additional cost and for many, a struggle, but, from your own post, you went in with a single recorder, having previously been told that it would not be allowed.
It may not sound it but I am sympathetic to your situation, but I don't have any solutions to the problem that you now have.

These would have been issues you could have raised if you had participated and an assessment was completed.
