Several years ago my PIP2 form/evidence got "lost" (after being signed for at the DWP). The DWP believed that I had never sent the form back to them. (I don't remember receiving any letters about it, but I was very mentally ill at the time.) I was still called to a f2f assessment, as they were aware that I had complex MH problems.
