You need to concentrate on two things;
First, the report but don't spend too much time on this. There's a guide to dealing with the report in the PIP section under appeals.
Secondly, go back through the form you completed. Whilst an inaccurate report is almost certain to have played a part you also need to consider whether you have properly explained his problems in the claim form, is it possible that you have misunderstood the Descriptors, described his problems in terms that do not match the Descriptors or failed to make appropriate use of the reliability criteria or the need for his limitations to be present on the majority of days to be considered?
Lastly, think about making a complaint to the Assessment Provider

Thank you Gordon. I don't think we could have been any more thorough with both the form and what was said at the assessment. I do know that I was there to support him and anything I added to what he said was completely disregarded and not recorded. She just said I didn't need to prompt his answers and agreed with everything he said. I actually had to expand on everything he said and explain further.

Sorry Gordon- basically the form and my statement seem to have been ignored and her report is based on her observations and assumptions, regardless of what was actually said.
