PA4 do individual documents have to be listed was created by Sarah
I have viewed the sample PA4 on here and that lists details of individual medical evidence considered
I.e Consultant rheumatologist report 12/10/10
This is my first ever PIP application. I have received my PA4 and it simply says All 44 pages of FME considered- Is there anything that states that these must be individually listed? Or that it is good practice to do so?
I’m not sure without counting pages whether that is all of the medical paperwork I supplied (forgot to number pages!!) it also makes no reference to the support letters supplied by my family and friends should it list these? Can they ignore them?
I’m trying to work it all out for a possible MR and becoming very confused. Things mentioned in the decision letter lead me to believe they’ve either not read the medical evidence, lost some of it or chosen to ignore it. An example being you’re not prescribed any supplements and have no specialist dietary input- my prescription list I sent shows that I am prescribed these things and my letter from my dietician was included so for me it would be extremely helpful to see the details of the evidence they claim to have viewed

DWP guidance requires all of the evidence to be listed but there is no legal requirement for it to be done.
If you feel that a piece of evidence not being listed may have materially contributed to your not scoring points then bring it to the attention of the DM but also explain why it is important, so you mention your dietary supplements, how do these contribute to your meeting a Descriptor?

Thank your for your reply.
I’m not 100% sure yet I’m becoming confused about the descriptors and whether I fit them or not (I am reading all the guides) but under taking nutrition I’ve said that I need prompting/reminding to eat (2d) because I forget but also because due to my M.E I sleep a lot so I’m woken for meals and depending how bad I am in terms of fatigue my food is brought to me in bed. If I wasn’t woken I wouldn’t get to eat for up two meals a day on some days.
The Assessor has ticked can take nutrition unaided and that because I eat at least one meal a day what I’ve said about needing reminding/forgetting is inconsistent. She then goes onto talk about the physical task of feeding myself which I confirmed I can do myself
Then the DWP decision letter states that I do not have specialist dietary input and am not prescribed any supplements. I have a dietician because of problems with digestion (dietician letter submitted to them) I also have vitamin B12 every 8 weeks and Vitamin D and calcium prescribed by Dr (copy of prescription supplied to them) I can see that the decision maker has not viewed my form or evidence relating to this because it was supplied and instead relied upon the assessors Report who has failed to list my supplements in her report despite asking me about it during the call
I’m not sure what the decision maker is getting... Perhaps that I do not have malnutrition?

It strikes me that the supplements are not really related to your needing prompting to eat, at best they would be treated as medication and therefore would be outside the scope of this activity.
To score, you need to concentrate on showing that you need the prompting. So, I understand why you need the prompting but what happens if you do not receive it? Do you not eat that day? Do you feel hungry, if you do does that not prompt you to eat?
If you regularly receive the prompting then it can be difficult to show that it is a necessity as you may actually be well fed but your not being malnourished does not mean that the prompting is no longer needed.
Remember, this activity is only about your eating and drinking, don't include information about any difficulty you would have with preparing and cooking the food, you won't score for it and including the info will detract from the rest of your arguments.

Thank you.... I will have to look at this again

Would it still be okay for me to correct that information in their report about not having supplements and dietician input? because regardless of the descriptor the report is not accurate
Perhaps I should focus on another descriptor as I don’t seem to meet this one correctly
This is me revisiting the descriptors for my MR to understand if I’ve been scored correctly which by the sounds of it for this one I have been. It’s my first ever application and I think I’ve misunderstood some of it
