Thank you. Ok phone call done today. Why do they ask you your address and phone number 3 or 4 times?! I've been sent an email with a link to complete the form online. It gives me a month to do it, but. Gordon, do you think it would be advisable to get the form completed and sent back earlier? (ie before my retirement date).
My main concern is not very much evidence to attach. You know what waiting lists are like - I've been trying to get a specialist referral for over two years. GP surgery just won't do it. It's like - go away and get on with it. Recently I pushed for it again as I am so much worse. And now have new MRI scans due next month (but too late to send the results with the form). Very little seems to be documented these days. I was sent to physio just over a year ago but have nothing in writing about it.
When doing ESA claims I've usually included a report or letter from a specialist, or at least from a GP. I think I'll have to ask my GP for a letter (but that could take time).

I've clicked on the link in the email which takes you to an online form process, similar to other .gov online forms (eg passport application). I don't know what happens if I start it - do I have to do the whole thing in one go? Or can I save it part way along and come back to it? It doesn't say anything about that.

It's a very annoying form. Having filled out a list of disabilities, medicationa and treatment and hit next, it said do you have any other medical conditions. I remembered I'd missed one off so ticked yes, thinking it would let me add it, and it wiped everything clean to start over again!
The disadvantage I can see with the online form is, you can't see what's coming next, as you can with a paper form. Is there a print out somewhere that shows the whole online form and questions?

Hi Hazel,
I would just add another snippet to all the good advice you have been given. Normally we say take care to carefully craft your submission, send in as much evidence as you can etc etc. In this case I would say that the most important thing is to get the form back at least a week before the due date. (And send it with a signed-for delivery and keep a copy of what you have submitted.) Provided you can evidence that you have returned your form in time there should not be a problem. If you have to throw yourself on the mercy of the DWP to overlook a late return that might be trickier (although I have known them to do so, even in a close to the deadline case like yours).
Obviously if you can send in a great submission and get it there on time then perfect!
Make sure you tick the right boxes, do the best submission you can in the time available, but if it is simply something like 'my disabilities do not allow me to do this because of the pain it causes' go for it. You can give all the additional information at your assessment and further down the line if needed.
CAVEAT: This is not generally the best advice - but this is an exceptional scenario!

I would also add a couple more things here -
Many people are reporting severe difficulty with actually getting thru on the phone at all. There is lots of anecdotal reports on this in the News section so be prepared to spend a LOT of time ringing, waiting in the queue etc…. In fact set aside the whole day as it’s so important you do get to speak to someone and you are on a bit of race against the clock with your application.
When you do eventually speak to someone they will probably tell you you’ll receive the form in a few weeks and blame Royal Mail etc…. but still only give you 4 weeks to complete and return it. Ask for an extension on the 4 weeks if you think you will need it so maybe put it in your diary a week after the original call to remind you.

If you can complete the form before your SP age then so much the better, but it should not make any difference.
What you need to do is take your time to complete the form to the best of your ability, quite often when you panic corners are missed and errors occur ....... so deep breath and do one question at a time.
use what ever supporting evidence you have that will help your cause, double check that what you send in will help you, DWP quite often say that appointment letters do not tell anything, it is the report after the appointment they want to see.
