Thank you for all your advice and support. I received a further extension to return my form. I used the guide you suggested to put my answers together to the questions. Do I need to state how often I do each activity in terms of the amount of days I can do it in a month or a week or the number of times I can do it in a day? Previously I just used phrases like most of the time and words like typically rather than being specific. Thanks again for your advice.

I'm glad to hear that you have a further extension to return your form. Don't try and tie yourself down to too much detail. I would say for the majority of the time 4 or more days a week. If there is something that you only do once a month - but you would like to be able to do it 4 times a month = then you can still say for the majority of the time you are unable to do it. I would avoid saying how many times you do something in a day unless it is relevant - simply because assessors do like to jump on numbers and twist them. Exceptions might be. A person only prepares a meal twice a week because most of the time they are too tired, depressed or in too much pain.
