Hi..
I may not be looking properly. I know where to find help here with forms but have you actually got the DLA & ESA forms to download please? I need to help my Mum fill in her DLA form but can only do it over the phone. It would be much easier if I had a copy of the form in front of me. I also need both the DLA & ESA forms to look at for myself. I was sure they were on here but can't find them.. Thanks v much & best wishes...

If it is a fresh claim for ESA, you can download an ESA1 claim form here :

If it is the ESA50 Questionnaire, you can download one from the link on this FAQ page :

Thanks so much. I'm waiting to be transfered (or not) from incapacity benefit so would it be ESA50 questionaire for me?
