Hello,

My ESA form arrived this morning. I am being reassessed from incapacity benefit. I am quite confused about the form, and do not have the aid of my carer right now.. and I don't believe she will be any less confused about this situation either.

I've looked through the website, and I am not quite sure where to go for help with filling out the form/explanation of the form? Could I be pointed in the right direction and perhaps get some advice? I feel quite alone and quite scared about this.

Thank you so much for the links, and linking the PDF version of the form in particular. This makes things a lot easier as I get confused a lot, and I did not even know this existed. I am really relieved I can do it this way. I do have a carer that can help me fill out the paper form, but I really prefer to use my own words when I can.

May I ask whether I just need to staple this together after it's printed out, and send it back in the ATOS envelope? Would that be accepted?

Yes, it is perectly accptable to send this back in the envelope provided. Make sure that your name and NINo are clearly maked at the top of each sheet and that you have numbered the sheets '1 of 20' etc

Hi D,

I'm glad that the links were of assistance. :)

Yes you can staple the pages together if you wish, and add extra pages and evidence such as a letter of support from your G.P. etc.

You can then post it back to ATOS in the envelope provide, if it will all fit in it, see :
