I want to try and give something back to this site as I have gained so much from the guides, the mods and other members' questions and experiences so I hope this may be of some assistance to someone.

I submitted a duplicate copies of my ESA claim to the DWP DM and ATOS. I did not ask any of my medical professionals for specific reports, instead i paid for a copy of my full notes from my GP and asked for all notes from one of the hospitals I attend (these were provided free).

I then spent weeks going through the lot and pulling out anything I felt was relevant to my claim - letters, MRI reports, diagnoses etc. I have several conditions so I grouped reports together accordingly.

I submitted 15 extra types pages which were answers or continuation of answers from the ESA 50 and clearly cross referenced them both with the ESA question number and the medical evidence. I numbered every extra page I included, starting at 21 (as the ESA 50 was on pages 1 - 20) . Including medical evidence I submitted a total of 61 extra pages. I tried to ensure every single page was necessary and relevant.

I sent them in a firm plastic booklet from Tesco with 80 clear pockets and put each sheet back to back. At the front I included a clear 2 part index, the first part for the ESA50 itself and additional sheets needed answering the questions and the second with a list of my 6 conditions and where to find the medical evidence for each one. Where there were overlaps, i submitted the same piece of evidence in more than one section.

For completeness i just wanted to say I received written confirmation from the DWP of my placement in the support group in today's post. I will now do anSAR to find out as much as I can with regards to which condition(s) qualified me for the support group. Many thanks again.
