As far as we are aware she never had an assessment for ESA although she remembers having a chat with someone on the phone from the jobcenter.
did she have to attend Work Focussed Interviews ?
No, never attended anything like that.

If the fit for work decision that you succefully appealed was the first time that your wife was assessed under The ESA WCA after being transferred over from IB, then this previous advice stands :
"She would still be entitled to the difference between what she has been receiving at Assessment Rate ESA whilst appealing,(Approx £71) and the same amount that she was receiving before being transferred from IB.
This would be owed from the date of the fit for work decision, until the date that her 365 days entitlement to CB ESA was exhausted.
If she has definitely exhausted her 365 days entitlement to CB ESA then this advice stands with respect to gaing access to The SG :
"With respect to your question regarding The SG, if she has exhausted her 365 days entitlement to CB ESA, and is placed into the SG as the result of a future reassessment instigated by the DWP or as a result of a review/supersession requested by your wife due to a change or deterioration in her medical conditions, payment of CB ESA in The SG would only be reinstated from the date of that decision, no backdating to the time of any previous decision.
Payment of CB ESA would remain in place, for as long as she stayed in The SG.2

Hi B,
The reason that I am assuming that the fit for work decision may have been your wife's 2nd ESA assessment, is that many people are placed into The WRAG or SG of ESA without the need of a face to face assessment.
However to find a claimant fit for work, a claimant must first have a face to face assessment.
I am really struggling to advise you properly, as it seems that you do not know when your wife was transferred over to ESA from IB, whether this last face to face resulting in the fit for work decision was her first ESA assessment, taking into account that a claimant can be assessed using paper evidence only, etc, etc.
Another indicator of how many assessments your wife has had, is, how many ESA50 Questionnaires has she had to fill in and return to ATOS ?

Could well be MHB, however B states that this was "many years ago"
IB to ESA transfers only began in March 2011.
