Hi M98,
Send a copy of the report to the same address as you sent your ESA50 to.
Include a covering letter stating that you wish this extra supporting evidence to be taken into account along with your ESA50.
If it turns out that you have to attend a face to face, take a copy of it with you to the assessment, and hand it to The ATOS HCP at the start of the assessment.
