Hello,as it appears i do not meet the criteria of descriptors but my GP flatly refuses to discuss my being signed off and has given me a letter 'to whom it may concern this man is chronically ill and not fit for any work'add to this that i need to take 500 mg+ of tramadol daily,how do i stand regarding being 'fit for work'under medication?i am trying to locate your form EA2010 but not very computer literate,much obliged ME

EA 2010 is not a form, it stands for Equality Act 2010 :

Reasonable Adjustments for Disabled people are covered under sections 20-22.

To be covered by EA2010, you must fulfill the definition of a "Disabled Person" :

www.gov.uk/definition-of-disability-under-equality-act-2010

Any formal request for Reasonable Adjustments should be made in writing, giving details of why they are needed, and citing EA 2010.
