I had similar chaotic problems when I first claimed ESA back in 2008. The one thing I say to everyone is ANY post should be sent special delivery (keep the receipt!).
When they receive the Special Delivery post all the details have to be logged down in the postroom. They also have to note where they passed the post to within the organisation. So making the excuse that they haven't received your forms becomes a total non-starter.
