Is this Esa 113 questionaire only sent to a GP, or could it be sent to a physiotherapist or other treating HCP that was last to assess you

Hi B,

It states on page 4 of the document on the link :

"Professionals involved in the patients care"

I am only aware of them being sent to a claimant's G.P., and that's not done that often.

G.P.'s are contractually obliged to fill in and return such DWP forms, although I have read elsewhere that they don't always comply with this obligation.

I suppose one could try insisting in the ESA50, that if an ESA113 was to be issued, that it should be to another treating HCP rather than the G.P.

Whether the DWP/ATOS would comply with such a request is anybody's guess.

Hi B,

If you read the ESA113, it is unlikely that they would send it to anybody other than one's G.P. due to the questions being with regards to a patients "General" medical history and conditions.

Another treating HCP such as a Physio, may not have access to your medical records, which would be required to fill in the ESA113.

Hi B,

Don't forget, it is a patient's G.P. who initially refers them to a Consultant, Physio or other treating HCP.

These referrals, and any resulting reports from the other treating HCP's will be in your G.P. records. Therefore, your G.P. has access to all the general information.

If one is receiving treatment from another HCP, there is nothing to stop you requesting an individual letter of support from them that pertains to the particular condition and resulting limitations that they are dealing with.

This can then be included with the ESA50.
