Severe Disability Premium

To receive the Severe Disability Premium you need to be living alone or considered to be living alone AND receiving an Income Related benefit such as ESA(IR) in whole or in part. There's no assessment, just a form to fill unless there are some concerns that you may be living with someone else in which case you may need to provide additional documentation such as a rent book.

Do you mean the IS10 form?

If it is an IS10 can you tell me what you are specifically struggling with as most of the questions should be fairly easy to answer.
