But I am still not sure which address to send it to. The pre-paid envelope that came with it is Wolverhampton but the reminder letter chasing the form says t send it to Wembley. Customer services (centre of Health and Disability) at Cardiff said use pre-paid envelope address and ignore the fact that their letter/reminder chasing ESA50 says send it to Wembley. But not sure this Cardiff customer service person knew much (re your previous post disagreeing with what they told me about having to return the ESA50 form ten days before the deadline date). Help please!

The ESA50 is returned to Maximus, they will look at it and decide on an assessment and once this process is completed they will return the claimants file to the DWP with the ESA50, any thing attached to it, anything presented at the assessment, any reports that they have collected and the assessment report.

Trying to go out is one thing, doing so is different, so which did you write

If you said you actually go out this frequently then I would be surprised if you get a home assessment.
