We have 2 front burners at almost eye level, so frying and boiling at eye level cannot be performed in a safe manner 365 days a year.

So you seem to have a good grasp of the issues and the criteria that you need to meet, that's what you need to put down on the form.
Think about aids that may enable you to cook, for example they will look at whether you could use a microwave.
